2012, Regulating Law No. 8968 (only available in Spanish here) ('the Executive Decree'), as amended by Decree No. 40008-JP (only
available in Spanish here) ('the Decree'). This agreement provides information on how you could exercise your data privacy rights, which
includes:

By signing below, I hereby authorize the Company to:

(1) obtain the personal and sensitive personal information indicated in this form.
(2) collect, use, process, share, retain, and dispose said information for purposes related to my employment application, which includes
recruitment and screening of prospective employees, administration of compensation and benefits, performance management, trainings,
career advancement planning, workplace management, workforce and risk management, occupational safety and health, and other
purposes stated in the Company’s Privacy Policy; and
(3) retain adequate documentation of said information ten (10) years from termination or resignation.
